Why a DOT Physical Matters Before OTR
The DOT medical physical is a federally required exam performed by a certified medical examiner on the FMCSA National Registry. It confirms you are physically fit to safely operate a commercial motor vehicle before you ever take a long-haul load.
- Federal law (49 CFR 391.41) requires a valid Medical Examiner's Certificate before you operate a CMV interstate.
- Long over-the-road (OTR) runs demand the stamina, vision, and reaction time the exam verifies.
- Undetected conditions — high blood pressure, sleep apnea, diabetes — are leading causes of preventable CMV crashes.
- A current med card keeps your CDL and employer's safety rating in good standing during roadside inspections.

What the DOT Physical Covers
Per FMCSA 49 CFR 391.41, the certified examiner evaluates the following. Pass, and you receive a Medical Examiner's Certificate (med card) valid for up to 24 months.
Vision
At least 20/40 in each eye (with or without correction) and 70° field of vision.
Hearing
Must perceive a forced whisper at 5 feet or pass an audiometric test.
Blood Pressure & Pulse
Screened for hypertension and irregular heart rhythm.
Urinalysis
Checks for underlying conditions such as diabetes or kidney issues.
Physical Systems
Heart, lungs, spine, neurological function, and extremities reviewed.
Health History
Disclose medications, surgeries, sleep apnea, and prior conditions.
How to Set Up a C/TPA
A Consortium/Third-Party Administrator (C/TPA) manages your DOT drug and alcohol testing program — random pools, queries, and Clearinghouse reporting. You designate your C/TPA on the FMCSA Drug & Alcohol Clearinghouse.
Always speak with your C/TPA first to arrange services before sending them an authorization request.

Talk to your C/TPA first
Before anything else, contact a Consortium/Third-Party Administrator and arrange your services. Confirm pricing, random-pool enrollment, and Clearinghouse handling BEFORE sending any request.

Register on the FMCSA Clearinghouse
Create your account at clearinghouse.fmcsa.dot.gov using Login.gov. Owner-operators must register as both an employer and a driver.

Designate your C/TPA
Inside the Clearinghouse, go to the C/TPA designation section and authorize your chosen administrator to perform queries and report on your behalf.

Send the authorization request
Only after speaking with your C/TPA, send them the electronic authorization request through the Clearinghouse so they can accept and manage your program.

Stay enrolled & compliant
Your C/TPA keeps you in a random testing pool, handles pre-employment queries, and maintains records required under 49 CFR Part 382.
